Chance Drawing Will Raise Funds for the Miracle Fund and Holtz Children’s Hospital
Each year, we host the Golden Angels Gala, which helps raise critical funds for Jackson Health System. However, we did not host the 2021 Gala due to safety concerns surrounding the COVID-19 pandemic. Although we cannot get together in person, we hope you will participate in our chance drawing in support of our Miracle Fund, which supports Jackson’s greatest areas of need, and Holtz Children’s Hospital.

OFFICIAL RULES OF THE CHANCE DRAWING
This chance drawing is conducted by Jackson Health Foundation, whose principal place of business is 1500 N.W. 12th Avenue, Suite 1117, Miami, FL, 33136. All prizes have been donated for the purpose of this chance drawing. Prizes will be awarded at the location according to the following schedule: Drawing will be held on May 27, 2021, by a member of the Jackson Health Foundation. The winner does not need to be present to claim the prize. The winner must be over 18 years of age. Prizes are non-transferable and not redeemable for cash. All applicable state and federal taxes resulting from any award of prizes are the responsibility of the winner. The winner will be required to fill out appropriate tax forms for the Foundation.
THE COLLECTION is providing a 24-month pre-paid lease on a 2021 Audi Q5 (10,000 miles per year/Stock #307743) to be included in Jackson Health Foundation’s chance drawing. One hundred percent of the proceeds will benefit Jackson Health System. Photo is for illustration purposes only. License, taxes, insurance, excess mileage and all fees are separate and are the responsibility of the chance drawing winner. Certain restrictions apply. The vehicle must be registered in the state of Florida. This prize in non-transferable. Chance drawing tickets cannot be shared. No dealer or dealer agents are eligible.
